Benefits:

First, you must submit a written application to be considered for the program, and, if accepted, your annual membership dues will be payable either by cash, check or credit card. Once you’ve been accepted into the program and your membership dues are paid, you are entitled to the following each year your membership is active:

Two dental cleanings with doctor’s exam and consultation (adult/child prophylaxis)
X-rays (Panorex, full mouth series, bitewing or periapical)
Oral evaluations with hygiene instructions
Flouride as needed (no age restrictions)
Oral cancer screenings
Up to two emergency dental exams with x-rays

All other dental procedures at a 20% discount off standard fees (written financial arrangements are required before treatment is scheduled). Services include:

Crowns
Dental Veneers

Dentures & Partials
Fillings
Implants
Periodontal Disease

Preventative Care
Root Canal
Sealants

All this adds up to a cost savings of $604 per member for each year you’re a member of the program!

An additional fee may be charged for any missed, cancelled or broken appointment without 24 hours prior notice. Failure to comply with the terms of the Plan may result in termination of the Plan and forfeiture of any fee paid by patient or other third-party.
